• Contact the American Association of Retired Persons if you feel you've
been mistreated. Few people realize the considerable power of this
28-million-member consumer organization. Write to the executive director
of your local or regional chapter. The AARP can exert direct and
indirect pressure on a hospital's chief executive officer.
The new pricing system resulted from Medicare's establishment of about
500 different groupings of inpatient and outpatient medical procedures,
known as diagnostic related groups (DRGs). Until five years ago,
Medicare reimbursed a hospital for the cost of a procedure. Now Medicare
uses DRGs to determine the average length of stay for a procedure and
prices it accordingly. If the patient leaves before that "average
period," the' hospital makes money. If he stays longer, the hospital
loses money. Result: Average length of stay in hospitals has dropped
dramatically in the last few years.
On the positive side, DRGs have eliminated inefficiencies. Few patients
need to stay longer than the designated period. Stays longer than
necessary increase the chance of hospital borne infections. However, no
one should be discharged before it's medically wise.

Mistakes: Hospital stays can be dangerous. Some 36% of those in one

generally pay 20% of their hospital bill. In the case of plastic
surgery, a popular outpatient operation, hospitalization costs may not
be covered by insurance at all.
Psychological benefit: In the case of older people, in particular,
leaving the familiarity of their home and spending a night or more in a
strange hospital is traumatic. With outpatient surgery, they can be back
home within hours, thus limiting the potential trauma.
The most common procedures performed where you can usually (but not
always) leave after just a few hours of recuperation:
Tonsillectomy.
Minor plastic surgery. Abortion. Skin-growth removal. Vasectomy.
Hand surgery. Eardrum repair. Removal of a cyst.

Some cataract operations.
Outpatients do not have the constant medical attention offered by a
hospital stay. They should check out the following on their own:
Is the surgeon associated with a good hospital in the area? Can the
doctor perform the operation in that hospital?
Does the surgeon have admitting privileges with a good hospitality):1at
will allow you to be transferred there in case of complications?
Is the admitting hospital nearby?
Is the doctor's assistant qualified to administer anesthesia?
Is there a recovery room where you can rest for a few hours?
Is there a full-time registered nurse present during surgery and during
the recovery period?
In the case of a private surgical suite, is the facility approved by any
regional agency? (There is no government organization that regulates
them.)
Be aware that outpatient surgery may be undesirable if you have physical
problems like persistent hypertension or psychological problems. Also,
most outpatient clinics use only local anesthesia. If you insist on a
general anesthetic, you may find there's no alternative to a hospital
stay.
Do-It-Yourself Medical Tests: Self-diagnosis has been made easier by a
proliferation of do-it-yourself medical testing devices for a variety of
ailments. Among them:
Blood pressure: Stethoscope and cuff kit are available at any department
or discount store for around $20. More sophisticated devices, with
digital readouts: $100 and up.
Bowel cancer: The Guaiac Test, available through some local offices of
the American Cancer Society, analyzes for blood in the stool.
Pregnancy: A kit available at drugstores gives a moderately reliable
diagnosis immediately.
Baby breathing: This device monitors for irregular infant breathing to
warn against crib death. Can be rented from hospitals.
Urine tests: For diabetes. At drugstores, the cost is about $3.
Coming soon: Blood tests for anemia and cho1esterol1evels; electronic
dental hygiene kits; emergency medical equipment similar to that used by
paramedics.
Diagnosing Diabetes: Diabetes is the third leading
cause of death in the country, after heart disease and cancer. The
disease is an inability of the body to handle sugar. It appears at two
times:
Juvenile: Strikes children and young adults (10% of the cases). Control:
Regular injections of insulin.
Maturity: Occurs in middle age and beyond.
More common and less dangerous. Control:
Weight loss through diet and exercise. Usually, oral medication or
injections are required.
Who gets it: The risk of diabetes in adults doubles with every ten years
of life and every 20% of increased body weight. Women run a greater
chance of contracting it than men. Many victims have diabetic relatives.
The symptoms in children: Rapid weight loss, fatigue, nausea, abnormal
thirst and hunger. In mature adults, if uncontrolled: Weight gain or
loss, tiredness, scaly or itchy skin, circulatory impairment.
Caution: For some adults, the disease is only uncovered during a routine
physical examination.

insurance plan. In some states, Blue Cross/Blue Shield won't cover
