How do you know when your occasional insomnia has become a chronic
disorder?
"To be a chronic problem, the loss of sleep must have a real effect on
your daytime functioning," says Dr. Zorick. "My usual question is, 'What
would you be doing differently during the day if you were getting eight
hours' sleep at night?' In other words, what benefit would being able to
sleep bring them?"
Sometimes, Dr. Zorick will find that a person is simply not sleeping as
much as he thinks he should, but that his daytime functioning is not
adversely affected. In those cases, the patient probably is trying to
sleep more hours than he needs to.
Dr. Zorick's prescription for the occasional insomniac: Condition your
sleep environment. Learn to associate your bed and your bedroom with
sleep.
How to do it? Dr. Zorick suggests:
Pay attention to bedroom conditions, such as light, heat, noise. Shut
off telephones if necessary. Keep temperature cool (around 68°). Make
sure your mattress and your sleep clothing are comfortable.
If you don't fall asleep right away, get up, leave the bedroom, and go
do something else. Don't lie awake thinking about it. Staying in bed for
hours trying to get to sleep accentuates the problem. You begin to
associate your bed and bedroom with trying to sleep instead of with
sleeping.
Stick to a regular bedtime schedule. Go to bed at the same time every
night-weekdays and weekends. Some insomniacs have the idea they'll catch
up on missed sleep on the weekends. You can't do it. Trying to do it
simply disrupts your biological rhythms.
As for other popular sleep inducers or aids, Dr. Zorick has this to say:
Sleeping pills. Doctor-prescribed sedatives are very useful in temporary
situations where a particular emotional or physical upset is the cause
of the insomnia. Problem: Tendency to become dependent on them, and a
worsening of the quality of sleep as more pills are used.

How to handle pills: Use for no more than a week or two. Expect that the
first night or two after stopping the pills will be very disturbed
sleep. That's perfectly normal. Expect it and accept it.
Nonprescription, over-the-counter sleeping pills are absolutely useless.
Studies have shown "sugar pills" to be just as effective.
Exercise. Early in the day is okay. Late in the evening is too
stimulating. Exception to the rule: Sexual activity, within a
comfortable relationship where no tension or anxiety exists, is helpful.
Caffeine. Coffee, tea, soft drinks act as stimulants. Avoid completely.
Alcohol. May help you get to sleep but interferes with quality of sleep.
Wears off after several hours.
Widely advertised insomnia cures like vibrating beds, prerecorded
cassette tapes, sleep masks? Dr. Zorick says, "Fine, if they relax you."
Side Effects Of Sleeping Pills: Hypnotics (drugs
prescribed to help you fall asleep and stay asleep) stay in your
bloodstream for a long time. If you take sleeping pills for a week, the
accumulation in your system will be four to six times what it was the
first morning. Be aware of any decrease in your abilities (such as
hand-eye coordination). Be especially careful about driving if drowsy.
They can interfere with other drugs you are taking. Let your doctor know
if you are taking another medication of any type.
If you have respiratory problems, impaired liver function, or you are
elderly, there are increased risks in taking Dalmane or barbiturates.
Pregnancy precautions: Barbiturates and chloral hydrate (one particular
kind of sleeping medication) both pass through the placenta to the
fetus. Dalmane and Quaalude taken by pregnant women have been linked to
birth defects.
