Answer: Remarkably few patients ask: What treatment is planned? What are
the alternatives? Costs? Method of payments?
Courtesy: If the dentist consistently leaves you waiting too long for
your appointments, talk it over. Your time is valuable, too.

What To Expect From A Good Dentist: A thorough examination, which
includes full-mouth X-rays (18 separate pictures) and a plastic
impression of your teeth so the dentist can study their shape and their
relation to one another. The dentist must probe the gums and also
examine the tongue and interior of the mouth.
Full cleaning of plaque deposits.
Clear explanation to the patient of how to maintain the health of the
gums, teeth, and mouth. (Even if patients say they know how.)
Description of what is going on in the pa-
tient's mouth and why. Also: What to expect in the future.
Effective recall system that brings the patient back to the dentist's
office regularly (at least every three months) for cleanings.
More than one treatment room and a strong support staff. Reason: A
dentist who personally does the cleaning and makes the X-rays is likely
to be inefficient (and also may be charging more for cleanings and
X-rays).
An orderly schedule. For some kinds of intensive work, be prepared to
spend a good many hours at the dentist's office. The dentist who can
take you any time for regular work without regard for efficiency may be
careless in his professional work.
The dentist spending some part of every week either teaching. in a
hospital, or in another clinic situation \\-here he's kept in touch
\\'ith other skilled dentists, new techniques, and experimental
procedures.
Dental Do's And Don'ts: Toothbrushes. Use two or more in rotation so
that they can dry out properly. Soft nylon is best. Natural bristle
brushes take longer to dry. If not used properly, they can damage gum
tissue because bristles are too firm and coarse. Angled brushes may help
in reaching some areas. Caution: Too-vigorous brushing can wear grooves
in tooth enamel. When used correctly, a toothbrush will not abrade
tissues or teeth. And hardness of the bristles is not as significant as
the way the brush is used and the time spent brushing.
Dental floss: Unwaxed floss is better because it absorbs particles.
Flushing devices: If used with too much pressure, device can damage
tissue, force debris into periodontal pockets, and cause inflammation
and infection. Use at half the recommended pressure.
Pain perception is less in the morning than in the afternoon, according
to recent research. Suggestion: Schedule dental appointments early in
the day.
Avoid over-the-counter tooth whiteners.
Reason: They contain highly abrasive ingredients that remove tooth
enamel after prolonged use.
A tooth knocked out in an accident can be saved. When a child falls and
loses a tooth, wrap it in a wet cloth, and bring it and the child as
quickly as possible to the dentist. Reimplantation works best with
children (sometimes works with adults, too).
Playing the trumpet or trombone can correct a bad bite. However, playing
flute or piccolo can make it worse. Saxophone can work either way.
Bad teeth don't cause headaches, bursitis, or anything like that. But
jaw and tooth pains may be "referred" pains that originate in other
areas.

Emergency Toothache Remedies: • If immediate dental
help is not available, take aspirin and try sucking on an ice cube. If a
filling is lost, put oil of clove on a small piece of cotton and pack it
into the cavity. Common mistake: Applying heat. Reason: If the pain is
the result of an abscess, heat can help to spread the infection.

Doubleday, New York.
• Try rubbing ice between your thumb and index finger on the same side
as the aching tooth. Massage until the area feels numb or seven minutes
have passed, whichever comes first. The ill tense cold often is as
effective as acupuncture or novocaine.
Preventing Gum Disease: Gun disease causes teeth to fall out as it
destroys roots and supporting bones. About 90% of tooth loss in people
over 40 years old is due to gum maladies.
Symptoms: Gums bleed easy-. Gum tissue swells.
Teeth become loose and move apart.
Cause: The irritation of gum and bone by plaque, a sticky, hard-to-see
film. It's made up of decomposed food and living bacteria, which must be
cleaned from the teeth every 24 to 36 hours.
Best means of plaque removal:
Brushing. Do it regularly with a soft or medium nylon toothbrush.
Concentrate on the juncture of teeth and gums. Hold the brush at a 45
degree angle. Use short, horizontal (almost circular) strokes. Don't
stop brushing if bleeding begins. Plaque removal reduces bleeding.
Flossing. Clip off an l8-inch length of unwaged dental floss, and curve
it around a tooth's edge. Bring the floss around the tooth until it
meets resistance from the gum. Scrape the tooth's edge from the top to
the gum at least three times. Repeat for every tooth surface. Be
patient: Flossing is tricky. It takes practice to do it quickly and
effectively.
Advanced cases of gum disease require surgery to remove pockets where
food and bacteria collect.
Tooth-Decay Culprits: Cream-filled chocolate cookies cause 50% more
cavities than pure sugar, according to a National Institute of Dental
Health Research study on laboratory animals. Runners up: Sugared cereals
and sugar-coated candies. (These have the same cavity-causing rate as
pure sugar.) Worse than sugar: Potato chips (84 %), caramels (73 %),
chocolate bars (72 %).
No surprise: All sugar-containing foods cause tooth decay to some
extent.
Of interest: Dica1cium phosphate (a food additive that improves
consistency or reduces tartness) reduces sugar's cavity-creation
ability.
Good news: Research with animals shows that cheddar cheese retards some
sugar-producing bacteria and acids that cause tooth decay.
Getting Teeth Capped: Capping is an elaborate,
dramatic, and expensive technique in dentistry.* Most people who are
concerned with keeping their teeth will have several crowns in their
lifetime. Teeth are capped when there has been so much decay or so many
fillings that there isn't enough tooth structure left to hold fillings.
Aesthetics is another good reason for capping.
Crowns are used to construct fixed bridges (rather than removable
dentures) to replace teeth that have been extracted.
First consideration before capping: Finding an experienced dentist. Caps
that aren't done right can accelerate the breakdown or loss of teeth.
Main problem: A somewhat increased difficulty in maintaining gums. The
margin between the tooth and the cap becomes an area a little more
susceptible to the formation of plaque and inflammation of gums. The
point:
Having caps means that more effort is required in maintaining gums.
Fit: Ideally, the cap should end above the gum. This makes it much
easier to clean. Problem: Having cap margins visible isn't as
aesthetically pleasing as having them invisible. A dentist may take this
aesthetic consideration into account where front teeth are concerned.
But where such small differences can't possibly show; it's a mistake to
insist on caps whose margins are shoved under the gum since this will
make proper maintenance of gums much harder.
How long caps should last: Caps last anywhere between five and 20 years
or more. In a relatively healthy mouth, properly maintained, they last
indefinitely. Porcelain or gold material can break down, requiring a new
cap in less than 10 years. If treatment is unsuccessful: The tooth can
be lost because too much bone has been destroyed, or the tooth may decay
under the crown.
Most important variable: The way the patient maintains the gums. There
is no question that proper care can double or triple the life span of
crowns.
How to make caps look real: Each tooth should vary slightly in shape,
position, and color. The total composition should be one of balanced
variety, not perfect symmetry. Avoid the gleaming, artificially white
teeth that so many entertainers choose. Natural teeth are not white.
Teeth should conform geometrically to the shape of a head or emotionally
to the body type, sex, or personality of the patient. There is no such
thing as feminine or masculine teeth.
* Caps, crowns and jackets are synonyms referring to a restoration that
completely covers the tooth, usually a gold thimble or cap covered by
porcelain.
Orthodontia For Adults: Adults who need braces may decide to forego this
dental treatment out of embarrassment. However, more than aesthetics
must be considered. Neglect that results in loss of teeth is too steep a
price to pay.
Recent advances in orthodontia have produced devices that can be worn in
back of the teeth, where they are virtually invisible. If braces are
prescribed, check with your orthodontist on this lingual treatment, and
keep smiling.
Denture Problems Of Older People: The older person whose dentures hurt
too much to wear may need a change in diet and life style, not new
dentures.
Reason: A dry mouth and tissue fragility are the most frequent causes of
denture problems. They are caused by loss of body water, aggravated by
the use of tranquilizers, antihistamines, and diuretics. Depression and
calcium or protein deficiencies produce these changes too.
Solution: Add vegetable soups (with some meat), bran, and yogurt or
cheese to your daily diet to rebuild oral tissues. Boil meats and eggs
to make proteins easier to digest. Exercise and social activities help
by increasing blood flow and warding off depression.
